Responsibility
JOB SUMMARY:
Performs professional public health nursing services in a variety
of community settings for the MCHD programs assigned. Meets preventive health needs of population
including low-income, uninsured families and individuals, and promotes positive
health behavior. Operates
duties within the scope of practice designated by the Colorado Nurse Practice
Act.
ESSENTIAL JOB FUNCTIONS:

Has
the primary responsibility for delivering public health nursing care in
assigned public health program areas.
Identifies the physical, environmental, cultural, spiritual,
social, and emotional health needs of individuals and families through
systematic assessment utilizing professional nursing process skills, health
history, physical evaluation, and other diagnostic tools.
Develops
and implements a comprehensive care plan based on systematic assessment,
involving the client, family, physician, and clinical nursing associates on the
health care team.
Provides for continuity of client care (case management) by
appropriate referral and follow-up. Acts as
a client advocate.
Assure
completeness of all required documentation and data entry for the assessment,
diagnosis, care plan, actions, and evaluation of care provided.
Promotes the maintenance of health by teaching, counseling, and
employing appropriate preventive, treatment, and rehabilitative measures for
quality patient care.
Works with other professional disciplines in planning,
coordinating, implementing, and evaluating comprehensive health services.
Represents
the department, as a health resource person, in community health organizations,
programs, and activities.
Participates
in teaching to clients and community groups.
Performs
related duties as required.
Public Health Clinic Nurse:
Use
appropriate injection techniques using the “Five Rights” (person, time,
medication, dose and site).
Ability
and skill to administer adult, pediatric and infant vaccinations – injections.
Knowledge of adult,
child and travel immunization needs.
Recognize and
respond to a client’s adverse reaction after receiving an injection by
following the emergency procedure policy
Take
responsibility for maintaining and updating knowledge for all vaccine
procedures and immunization schedules and for all contraceptives and sexually
transmitted infections
Assist in the
development and implementation of public education campaigns to encourage
Receive and triage
phone calls regarding immunizations, communicable disease, possible
bioterrorism events, answering questions or referring to appropriate staff
Assist
members of the Communicable Disease/Bioterrorism team in the investigation of
any reports.
Assure that
appropriate lab tests and/or treatments are provided for client and contacts
and reported or lab tests and treatments are carried out as ordered by a nurse
practitioner or physician
Track and
follow up on specimens that have been sent to laboratories for processing,
being mindful of time factors in communicable disease control
Follow
established protocols for treatment of diagnosed disease. Refer client to their physician when beyond
scope of nursing practice or protocol
Knowledge of HIV
testing and counseling methods and procedures
Assist
members of the Communicable Disease team with investigations as needed
Performs other related duties as assigned.
Health Education Public
Health Nurse:
Assess
individual and community needs for health education programs, plan and
implement effective strategies and interventions for programs, conduct
evaluation and research related to health education.
Become
certified as Child Passenger Safety Technician and assist at Fitting and
Education Stations as scheduled.
Become
certified in Chronic Disease State Management, “Healthier Living”; and “Matter
of Balance”; and “SHAPEDOWN” in order to schedule, promote, teach and train
others to teach or assist in programming.
This position
requires some outside work in Fit Education Stations.
Oral and
written communications must be clear and effective.
MINIMUM QUALIFICATIONS
REQUIRED:
Education
and Experience:
Varies by department
and program confirm requirement with direct Supervisor or Director:
LPN,
RN or BSN graduate accredited by the National League of Nursing.
Current
unrestricted license in good standing.
Any
combination of education, training and experience which provides the knowledge,
skills and abilities required for the job.
Licenses and Certifications:
Current
license to practice as a Registered Nurse in
PHYSICAL REQUIREMENTS AND WORKING
ENVIRONMENT:
This position
requires standing, stooping, sitting, bending, twisting, and lifting up to 10
pounds.
Work is
generally confined to a standard office environment.
May be exposed to communicable diseases, infections, blood and bodily fluids.
