Resource navigators with Grand Valley Connects are adjusting their in-person availability at Mesa County Libraries to meet community needs. While many community members connect with our team by phone or through the website, walk-in services at centralized locations are especially valuable to those without reliable technology or transportation. 

“We understand how overwhelming resource navigation can be. We want to meet community members where they are and empower them to pursue a healthy and stable future,” said Fallon Knez, Grand Valley Connects Supervisor.

 

Walk-in hours available

Services are available on a first-come, first-served basis at the following locations and times:

  • Central Library at 443 N. 6th Street
    First and third Tuesdays, 12–2 p.m.
     
  • Clifton Library at 3270 D ½ Road
    Second and fourth Tuesdays, 12–2 p.m.
     
  • Health and Human Services Building at 510 29 ½ Road
    Monday–Thursday, 7:30 a.m.–5 p.m.
    Friday, 7:30 a.m.–12 p.m.

Community members can contact Grand Valley Connects by calling 970-683-2663 or visiting grandvalleyconnects.com. A simple referral form is available online to begin the process. 

 

What to expect during an appointment

When a community member sits down with a resource navigator, they will have a conversation regarding what needs they have. This includes a 10-15 minute intake to get the whole picture of what needs and resources might be available to the community member. Then they make a plan regarding next steps to connect to the appropriate resources. The resource navigator follows up regularly—at 30, 60, and 90 days.

 

Grand Valley Connects background

Grand Valley Connects offers personalized, one-on-one support to individuals who may be struggling to find help with essential needs. Resource navigators work with community members to assess their situations and connect them to services. These services include, assistance with groceries, utilities, and childcare costs, mental health counseling, health insurance enrollment, and employment and disability services.  Since its launch in 2022, the program has seen a steady increase in demand. In its first year, 300 community members reached out for assistance. In 2025, that number quadrupled to over 1,250 people.
