How to Request Child Welfare Referral Reports

Coming soon: Starting June 1, MCDHS will begin charging a fee for child welfare records and report requests. The standard fee will be $25, and urgent requests will cost $50. If you're requesting records and your income is below the poverty line, you may be eligible to have the fee waived by completing a self-attestation form. Fees must be paid in full upfront, either online or in person at our clerical offices, before we can process and release any records. Please stay tuned for more information.

The Child Welfare Assessment Investigating Case Manager has 60 days to finish assessments/interviews from the time the referral was taken and enter it into the computer system.

Child Welfare Referral Records can take 30 days after receiving the completed paperwork to complete the records request. Once the records are pulled, they are sent to the Mesa County Attorney Office for redaction. This may take up to an additional 2 weeks.

Former Foster Care Records can take up to 60 days to complete your records request, followed by an additional 30 days from the Mesa County Attorney’s Office for redaction.

To complete your request:

  1. Complete the Child Welfare Records Request Form (linked below). Be specific as to what referral(s) you are looking for.
  2. Page 2 will need to be notarized. (There are notaries at DHS for your convenience)
  3. Attach a copy of your Identification/Driver's License
  4. Email the completed form, current phone number, and a copy of your ID to dhs-cw-records-request@mesacounty.us

Once the Records Custodian has received the completed request form and copy of your ID, we will start your request and call you when your request is complete.
